Job Description:
This is an exciting opportunity for the right person to work with a range of professionals and people to support the principles of Home First. The worker must have the ability to work as part of a team in a pro-active manner identifying opportunities for Home First whilst ensuring the patient has the appropriate level of support to discharge from hospital. Tasks will be carried out in partnership with service users, family members, colleagues within Social Policy and other professionals
Experience and Qualifications:
SVQ 3 or HNC in Social Care or equivalent or relevant experience in a social or health care setting is essential. It is desirable for the candidate to have relevant assessment and care management experience in a social work practice team. The worker will be required to undertake essential training including in adult support and protection training. The worker must have the ability to travel throughout West Lothian. Before confirming appointment, you will be required to obtain PVG Scheme Membership through Disclosure Scotland.
