Job Description:
This is an exciting opportunity for the right person to work with a range of professionals and people to support the principles of Home First. The successful candidate will be based within the Integrated Discharge Hub - Hospital Social Work Team delivering a service to patients in St John’s Hospital to help facilitate timely discharge from hospital and support people to remain in the community, where they are medically well but need sort term support due to a change of circumstances.
Experience and Qualifications:
The worker will be required to undertake essential training including in adult support and protection training. The worker must have the ability to travel throughout West Lothian. Before confirming appointment, you will be required to obtain PVG Scheme Membership through Disclosure Scotland. SVQ 3 or HNC in Social Care or equivalent or relevant experience in a social or health care setting is essential. It is desirable for the candidate to have relevant assessment and care management experience in a social work practice team.
