Job Description:
The Integrated Discharge hub is an inter – agency multi – disciplinary team made up of hospital social workers, community care co-ordinators, reablement staff, discharge facilitators, AHP staff as well as representatives from different services.
This is an exciting opportunity for the right person to work with a range of professionals and people to support the principles of Home First.
Experience and Qualifications:
The worker must have the ability to work as part of a team in a pro-active manner identifying opportunities for timely discharge whilst ensuring the patient has the appropriate level of support, which may incorporate the use of technology upon discharge from hospital. The worker will be required to have a good understanding of Self-Directed Support and the importance of completing outcome focused assessments. The worker will be required to undertake essential training including in adult support and protection training.
The worker must have the ability to travel throughout West Lothian.
SVQ 3 or HNC in Social Care or equivalent or relevant experience in a social or health care setting is essential.
It is desirable for the candidate to have relevant assessment and care management experience in a social work practice team.
