Job Description:
As a member of the team, a primary function will be to provide short term supports to individual to return from hospital or to prevent admission, in addition to other aspects of service delivery such as:
Assisting individuals who have fallen but are uninjured
Responding to telecare alerts when individuals activate their pendant alarm or when no movement is detected in their home

Experience and Qualifications:
It is essential that candidate have the following:
Experience working in a community‑based care setting
Literacy and numeracy skills appropriate to the role
Knowledge of a wide range of health-related issues
SVQ Level 2 (Social Services and Healthcare SCQF Level 6) or willingness to work towards achieving it to meet SSSC registration requirements
A full driving licence
This position requires registration with the SSSC. The successful candidate will have six months from the date of commencement of this post to become registered with the SSSC. SSSC stipulate you must submit your application within three months of your start date. If you are already registered with SSSC you will be required to add West Lothian Council to your account
