Job Description:
The team is responsible for the assessment for new community services, review of existing care at home services and oversight of unmet need in relation to Care at Home. Assessments adopting a strength’s- based approach will be undertaken in a multi-disciplinary context with a focus on collaboration between all members of the team to secure the best possible outcomes for the people being assessed
Experience and Qualifications:
A Degree Level Qualification in Occupational Therapy or other approved/recognised Occupational Therapy qualification is essential as is registration as a occupational therapist with HCPC The successful candidate will need to have the ability to work in partnership with other statutory and third sector organisations and should have extensive experience within an adults or older peoples team setting The ability to travel to service user’s homes across the whole of West Lothian will be essential. Excellent I.T. and communication skills and report writing are essential
